CHICAGO (AP) - U.S. Sen. Dick Durbin says the findings of a report from a southern Illinois Veterans Affairs hospital are inexcusable.
A federal report released today shows inconsistencies with reporting deaths and problems with patient safety, including surgeons performing two procedures they weren't authorized to perform.
The facility in Marion has been under fire for years. Major surgeries have been suspended there following a spike in deaths two years ago.
According to the report, problems at the facility were wide-ranging. For example, three sets of documents showed three different death totals for April 2009.
The report also reveals outdated staff training and patient data analysis.
A hospital spokesman didn't immediately have comment.
